Improve the Script Fragments UI/UX#106

As a user,

I would like to be able to customise the header/title and configure name/note of the Script Fragments,

So that the users can see what this fragment is about and admins can determine and reference which fragments are which.

(I attached some screenshots for reference.)

This is definately needed. Just to be clear (as the above screen shots are from Data Center Jira where it works correctly), the way it looks now in Cloud is as shown in the following attachments - no way to identify which fragment is which and the pane is just named “Script Fragment” in the user view - which is meaningless to end-users.
